Paperwork & Payment

Your paperwork must be in the office within 4 business days of when you made your reservation or we have the right to give your spot to someone else.

Paperwork & payment instructions follow:

  • Print and complete all paperwork.
    • Emergency Contact Waiver Form (click to download)
    • Make sure you read the make-up and cancellation policy carefully
    • Make sure you initial and sign the second page of the Emergency Contact & Waiver form in the correct places.
  • Payment options and instructions.
    • If signing up for multiple sessions, you may write one check for the entire sum, or a check for each session
    • Mail a check to:

    • Call the office at 919-787-1518 x28 to pay by credit card
  • Mail your completed forms to the office. Once we receive your paperwork, we will send you an e-mail letting you know it arrived and that the registration process is complete.
